Patient-Centered Medical Homes (PCMHs) could play a part in reforming the U.S. health care system. A PCMH is a care facility that structures care around patients, often implementing coordinated care. CareFirst BlueCross BlueShield, the leading nonprofit health plan in the Mid-Atlantic region, has asked Westat to evaluate its PCMH program, which gives providers financial incentives to coordinate care for patients with chronic conditions.
- Westat is conducting the evaluation to assess the PCMH program’s implementation and its impacts on health care quality and cost. Westat is evaluating over 3,400 primary care providers in 400 PCMHs, comparing high-performing PCMHs with low-performing PCMHs to figure out the factors that make the difference.
- The evaluation uses a mix of qualitative data, such as interviews from PCMH staff and patients, and quantitative data, such as economic analysis of claims and administrative data from CareFirst and Medicare patients.
- We are producing reports to help explain program outcomes and make recommendations for improvements. These reports will help CareFirst improve its program and help health care providers implement structures to care for patients better.
